Step 2: Inform your doctorInform Your Doctor

If your doctor has not suggested the ReBuilder to you, or is not aware of your interest, contact your doctor's office and inform them of your interest.

Step 3: Get assigned a case managerGet Assigned A Case Worker

Your Inquiry form will be assigned to a case manager who will get to work on determining your eligibility, coverage and anything your insurance may require.

Step 4: Eligibility checkEligibility Check

Your case manager will contact you with the results of the eligibility check and see how you wish to proceed.

Step 5: Working with your doctorWorking With Your Doctor

Your case manager will work with your doctor's office to collect and process the required prescription and documentation for Medicare and/or your insurance. We do not suggest getting the prescription until your eligibility and insurance requirements have been determined. We will obtain up to 6 months of medical records to determine that you meet Medicare/Insurance qualifications. If the records meet the guidelines, we will contact you to visit you doctor for a prescription and for him/her to document that they recommend the ReBuilder. If your records do not meet the guidelines, we will notify you and inactivate the request.
