1. You fill out the medicare intake form so we know where to send your ReBuilder and how to reach your doctor, and the contact informaton for any secondary insurance coverage that you may have.
2. When filling the intake form, please include your payment information to pay your copay. Once your is verified, and we have received your intake form, we will immediately send your ReBuilder via FedEx and you will receive notice of the tracking number.
